Exam sections — what each tests

Biology / Life Sciences

MCQ80 questions80 marks
Human anatomy: skeletal, muscular, nervous systemsCell biology and geneticsHuman physiology (all systems)Biomolecules and metabolismMicroorganisms and infectious diseaseReproductive health
Strategy: Biology is the backbone of all physiotherapy entrances. The musculoskeletal system is uniquely important for physiotherapy — study all bones (206), joint types (hinge, ball-and-socket, pivot, gliding), and major muscle groups with origin/insertion/action. The nervous system (CNS, PNS, ANS, reflex arcs) is directly relevant to neurological physiotherapy — master it at NCERT level. Human physiology (all 10 systems) provides the clinical context for the career.

Physics & Chemistry

MCQ60 questions60 marks
Mechanics: forces, levers, torque (biomechanics basis)Waves: sound and ultrasound physicsElectricity and circuits (electrotherapy)Thermodynamics (heat/cold therapy)Biomolecules, pH, and bufferingOrganic chemistry reactions
Strategy: Physics and Chemistry for state BPT entrances are at standard Class 12 NCERT level, slightly easier than AIIMS. Focus on Physics chapters that directly map to physiotherapy modalities: waves (ultrasound, laser), electricity (TENS, NMES), thermodynamics (heat packs, cryotherapy). For Chemistry, biomolecules and acid-base pH are most clinically relevant. Complete NCERT exercises for both; state exams rarely go beyond NCERT.

General Aptitude

Aptitude40 questions40 marks
Logical reasoningEnglish comprehension (medical context)Basic arithmeticData interpretationSpatial reasoning (relevant to anatomy understanding)
Strategy: Aptitude sections in state physiotherapy exams are typically easier than AIIMS. Focus on English reading comprehension (medical passages are common), logical reasoning (series, analogies), and basic arithmetic. State CET aptitude is often similar to Class 10 level — don't overinvest here; Biology is where the exam is won. Practice 20 mixed aptitude questions daily for 3 weeks to reach consistent 80%+ accuracy.

Allied Health & Physiotherapy GK

Objective20 questions20 marks
Indian Association of Physiotherapists (IAP)NCAHP Act 2021 and its implicationsNational Health Mission (NHM)Disability rehabilitation policy (RPWD Act 2016)Sports Authority of India (SAI) programmesWHO ICF (International Classification of Functioning)India's disability statistics and rehabilitation gap
Strategy: Allied health GK is the unique differentiator in physiotherapy entrances. Study the NCAHP Act 2021 — it is the most significant regulatory change for physiotherapy in India, creating a national licensing framework. Know the Rights of Persons with Disabilities (RPWD) Act 2016 and its rehabilitation mandate. IAP's history (founded 1955), clinical practice guidelines, and accreditation standards appear regularly in JIPMER and PGIMER allied health papers.

Career paths in physiotherapy and rehabilitation

Orthopaedic Physiotherapist

Rs 25,000–50,000/month (entry level)

The largest physiotherapy sub-speciality in India. Post-surgical rehabilitation (knee replacement, hip replacement, ACL repair) is the dominant workload. Orthopaedic physiotherapy clinics near major hospitals in Bengaluru, Mumbai, and Delhi earn Rs 80,000–1.5 lakh/month for independent practitioners. MPT in Musculoskeletal doubles starting salary in private practice.

Sports & Performance Physiotherapist

Rs 35,000–1,00,000/month (league teams pay higher)

India's sports ecosystem is the fastest-growing physiotherapy employer. IPL, ISL, PKL, and PBL franchises pay Rs 80,000–3,00,000/month. Sports Authority of India (SAI) national camp physiotherapists earn Rs 60,000–1,20,000. Corporate wellness and gyms employ physiotherapists for injury prevention at Rs 40,000–70,000. This speciality has the highest earning ceiling in Indian physiotherapy.

Geriatric & Home Care Physiotherapist

Rs 30,000–60,000/month

India's population aged 60+ will exceed 350 million by 2050 — the fastest ageing demographic in the world. Home physiotherapy services are exploding: Portea, PhysioAge, HealingTouch, and urban physiotherapy startups pay Rs 40,000–80,000 to experienced physiotherapists for home visit caseloads. Post-stroke, Parkinson's, and post-fracture home rehabilitation is the dominant case mix.

Paediatric Physiotherapist

Rs 30,000–55,000/month

Cerebral palsy, developmental delay, and autism-spectrum rehabilitation are growing as awareness increases. NIMHANS (Bengaluru), Institute of Child Health (Kolkata), and Rainbow Children's Hospitals employ paediatric physiotherapists. MPT in Paediatrics from KEM, Manipal, or Amrita opens specialised roles. International career in paediatric physiotherapy is strong — Australia and Canada specifically recruit Indian paediatric physiotherapists.

Common mistakes that cost you a seat

Applying for only the most competitive BPT seats and ignoring the state domicile quota — 85% of government BPT seats are domicile reserved; applying in your home state dramatically improves your odds.

Underestimating the anatomy depth required — musculoskeletal anatomy for physiotherapy is more detailed than what NEET MBBS tests; study bone anatomy, joint types, and muscle origins/insertions as a separate focus area.

Not researching whether a college has IPC (Indian Physiotherapy Council) accreditation — BPT from non-IPC-approved colleges cannot be used for government employment or international registration; always verify approval status.

Skipping the NCAHP Act 2021 entirely — this legislation changed the entire regulatory landscape for physiotherapy in India and appears in GK sections of JIPMER and PGIMER allied health papers.

Expecting the same syllabus across all state exams — Tamil Nadu's TNEA, Maharashtra's MHT-CET, and Karnataka's CET have slightly different difficulty distributions; download the specific syllabus and previous year papers for each.

Assuming physiotherapy is easy to migrate to internationally without formal registration — AHPRA (Australia), HCPC (UK), and CPTBC (Canada) each require a skills assessment; start the registration process in BPT year 3, not after graduation.

Frequently asked questions

How long is the BPT programme and what does the curriculum include?

BPT is a 4.5-year programme including a compulsory 6-month internship in the final year. The first 2 years cover basic sciences (anatomy, physiology, physics, biochemistry, pathology). Years 3–4 cover clinical physiotherapy (musculoskeletal, neurological, cardiopulmonary, paediatric, sports). The 6-month internship is supervised clinical practice in each speciality rotation. After BPT, a 2-year MPT specialisation is available.

What is the NCAHP Act 2021 and how does it affect physiotherapy?

The National Commission for Allied and Healthcare Professions (NCAHP) Act 2021 created a central regulatory body for 56 allied health professions including physiotherapy. Before NCAHP, physiotherapy licensing was state-level and varied widely. NCAHP standardises qualifications, creates a national register, and allows interstate mobility of physiotherapy professionals. For international registration, NCAHP certification strengthens applications to AHPRA (Australia) and HCPC (UK).

Can I become a physiotherapist if I have PCM (without Biology) in Class 12?

Generally, no — virtually all BPT programmes require Physics, Chemistry, and Biology (PCB) in Class 12. PCM without Biology is not accepted because physiotherapy fundamentals require biological sciences. However, a few private institutions and open university programmes may have different criteria. If you have PCM, you can study Biology as an additional subject through open schooling (NIOS) and then reapply — this adds one year but opens the physiotherapy pathway.

Is physiotherapy a good career for someone interested in sports?

Physiotherapy is arguably the best healthcare career for sports enthusiasts — you work directly with athletes, train at the highest performance levels, travel with national and international teams, and have direct impact on competitive outcomes. Indian sports physiotherapists for cricket, hockey, and football teams travel for all overseas tours. IPL and ISL physiotherapists watch every match from the dugout. Unlike sports medicine doctors (who need MBBS + additional 5 years), BPT + sports MPT is achievable in 6.5 years total and provides comparable proximity to professional sports.

What is the difference between a physiotherapist and a chiropractor?

Physiotherapists are registered allied health professionals with 4.5-year BPT degrees, trained in exercise therapy, electrotherapy, manual therapy, and neurological rehabilitation. Chiropractic is a separate profession not formally regulated by Indian law — NCAHP does not recognise chiropractic as an allied health profession in India. Physiotherapy is the evidence-based, regulated, internationally portable profession for musculoskeletal and neurological rehabilitation in India. Chiropractic practice in India exists in a legal grey zone and is not recommended for patients with serious conditions.
